I need help⚠️

Recently my skin has been breaking out a lot I have had many pimples coming and going also blackheads I have oily skin and I usually use cerave foaming cleanser along with moisturiser but it hasn't been working the same as it used to how can I get my skin clear? I also have red patches that look like rashes including a few scans from past spots
